For your first visit, please bring the following:

   1. A current list of medications and medication allergies

   2. Your insurance card

   3. Your eyeglasses and your contact lens boxes

Expect that your eyes will be dilated, so please also bring a pair of sunglasses to wear after your appointment and someone to drive you home, if necessary.

If you are a minor, please bring your parent or guardian.
